Here are the next steps for booking with Rev Julienne for a memorial.

Your agreement with Rev Julienne

I agree to hire Rev Julienne Givot to officiate a ceremony for my loved one on the date listed above, with further details arranged by email. Compensation for services has been quoted to me and will be confirmed by email after this form is submitted. A $50.00 deposit will then be requested to complete the booking. The remaining balance is due two weeks before the ceremony date, or at booking if the ceremony is less than two weeks away.

Cancellation and rescheduling: I understand that the deposit is applied toward the total fee, and that the deposit and any fees paid are non-refundable if the ceremony is cancelled. If plans change, Rev Julienne is happy to reschedule to a new date within one year of the original ceremony date, based on her availability. Extensions beyond one year may be considered on a case-by-case basis.
