  • Complete this form & click send to initiate booking with me.
  • I will email you to confirm that the date and time is still available and if you are booking ceremony only, I will request a $50.00 deposit. If you are booking an elopement package, I will confirm the date and send you a separate booking & deposit form for that.
  • Once the deposit is received I will confirm by email and we are all set.
  • A wedding questionnaire will be sent 1 month in advance of simple weddings and 3 months in advance for custom weddings.
  • The remainder of the fee is due 2 weeks before your date

Here’s what happens next: 1) I’ll reply to confirm your date and time, along with the total fee. 2) I’ll request a $50 deposit to secure your booking. 3) Once the deposit is received, I’ll send your final booking confirmation. Please allow up to one business day for a response (usually much sooner!).

Your agreement with Rev Julienne: We agree to hire Rev Julienne Givot to officiate our wedding/renewal ceremony on the date, time and location listed above. Compensation for services have been quoted to us and will be confirmed by email after submission of this form. A $50.00 deposit will be requested thereafter to complete the booking. We agree to maintain open communication to ensure a meaningful and memorable ceremony.
