I’ll email you to confirm your date and time are available.
Officiant service: I’ll request a $50 deposit directly.
Elopement package with photography: I’ll send a separate booking and deposit form.
Once I receive your deposit, I’ll confirm by email and your date is officially booked. Your remaining balance is due 2 weeks before your ceremony date.

Your agreement with Rev Julienne

I agree to hire Rev Julienne Givot to officiate my wedding/renewal ceremony on the date, time and location listed above. Compensation for services has been quoted to me and will be confirmed by email after this form is submitted. A $50.00 deposit will then be requested to complete the booking. The remaining balance is due two weeks before my ceremony date.

Cancellation and rescheduling: I understand that the deposit is applied toward the total fee, and that the deposit and any fees paid are non-refundable if the ceremony is cancelled. If plans change, Rev Julienne is happy to reschedule to a new date within one year of the original ceremony date, based on her availability. Extensions beyond one year may be considered on a case-by-case basis.

My responsibilities: I agree to:

For weddings, obtain a valid Hawaii marriage license before the ceremony and have it available on the day. (Not required for vow renewals.)
Request any change to the ceremony time or location as soon as possible, including weather-related changes. I understand that changes are subject to Rev Julienne’s approval and availability.
Maintain open communication to ensure a meaningful and memorable ceremony.
